Donation Requests

Windy City Smokeout is pleased to donate wristbands to charitable fundraisers and events.

Please read this entire page before submitting your Wristband Request form below. Organizations requesting wristbands must meet the following requirements:

• Organizations must be within a 75-mile radius of Chicago to be eligible for consideration
• Only one (1) form will be reviewed per organization.
• Submitting a wristband request form does not guarantee a wristband donation
• Wristband request status updates will be sent via email to the primary contact person listed on the form
• Wristbands must be picked up from Will Call under the name of the wristband winner.
• The primary contact person is responsible for ensuring that the winner fills out the form to have the tickets placed under their name.
• Requests for Windy City Smokeout 2025 tickets must be placed four (4) weeks before the charitable fundraiser or event and before May 15th, 2025.

All charity wristband requests are generally reviewed within two (2) weeks upon receipt; organizations will receive an email response once a decision has been made
